A privacy breach occurs when there is unauthorized access to or collection, use, disclosure or disposal of personal information. The most common privacy breaches happen when personal information is lost, stolen or mistakenly disclosed – for example, when an email containing personal information is mistakenly sent to the wrong person.  

Upon completing this form, the Privacy Officer will work with you and your administrator or manager to respond appropriately.
